Reimbursement for Internet Access
ISP Reimbursement - To be reimbursed for your Internet access expenses (up to the cost of the lowest level of broadband service available in your area) please download and print the ISP Reimbursement Form. The current billing period is from January through May 2009. Then submit the completed form by mail or in person, along with your original receipts for Internet service from January through May 2009 to:

Please note:  You must send in your original billing statements. Please keep a copy of the original statement for your records. No reimbursements can be issued without submiting your billing statements. No exceptions will be made to this policy.

All reimbursement forms for the second semester must be turned in by June 12th, 2009. Checks will be issued by July 1st, 2009.
